Experimental Pyelonephritis by Retrograde Infection


Abstract
To investigate the pathogenesis of pyelonephritis by retrograde infection, morphological changes of kidney were observed after single infusion of E. coli(5 x 108/ml) into urinary bladder of the rats.
The following results were obtained
1 . In the group of infusion of E. coli into the urinary bladder without urethral ligation, significant gross and histopathological changes were not recognized.
2 . In the group of urethral ligation for two hours after infusion of E. coli into the urinary bladder, most of them showed development of acute pyelonephritis.
3. When retrograde infection had been induced, spreading of infection were developed through the parietal epithelium covered fornix, chiefly. And the inflammation of subparietal epithelium was spread to perihilar cortex, thereafter to cortex & medulla of greater curvature and to papilla. The spread of the inflammation was attained via the renal tubules, chiefly.
From these results, it is offered that retrograde pyelonephrtis was not almost developed in the condition of normal urethra and bladder, but when the pyelonephritis was induced by ligation of urethra, involvement of kidney was initiated in the parietal epithelium covered fornix, and spread of infection in renal parenchyma was attained by pattern of contiguity via renal tubules.
